冠心病经皮冠状动脉成形术前后QT离散度的对比研究

目的 评价PTCA对冠心病电稳定性的影响,并比较不同冠脉病变支数和不同冠心病类型对QT离散度 (QTd)的影响。 方法 对78例冠心病经皮冠皮状动脉成形术 (PTCA)前后QT离散度的对比观察。结果 急性心肌梗塞 (AMI)组QTcd与稳定型心绞痛 (SAP)组相比有显著性差异 (P<0.05) ;不同支数冠脉病变QTcd相比均无显著性差异 (P>0.05) ;冠心病经PTCA后QTcd明显减少 ,与PTCA前相比差异有极显著性 (P<0.01)。结论 QT离散度与冠脉病变支数无关 ;AMI时QTcd明显增大。冠心病经PTCA前后QTcd差异极显著 ,ATcd明显减少 ,可作为PTCA后心肌再灌注成功的指标 ,能明显改善冠心病的预后,是否可作为冠脉再狭窄的早期指标 ,有待于进一步观察。

Objective To explore the effect of percutaneous transluminal angioplasty ( PTCA ) on QT dispersion of the coronary heart disease. Methods 78 coronary heart disease cases were treated with PTCA to examine their QTd. Results Significant difference was observed between the QTd of the acute myocardial infarction cases and that of the stable angina pectoris cases ( P< 0.05).But no statistical difference was revealed between the different diseased arteries (P>0.05 ). After the PTCA intervention, QTd was greatly reduced and significant statistical difference was observed between QTd before PTCA treatment and QTd after the PTCA treatment (P<0.01 ). Conclusion Qtd could be used as an index for evaluating the myocardial reperfusion after PTCA.
