肾上腺髓性脂肪瘤诊治体会

Diagnosis and treatment of adrenal myelolipoma

目的 探讨肾上腺髓性脂肪瘤 (AML)临床特征和诊治方法。方法 对 2 0年间收治的 13例AML病例资料进行回顾性分析。结果 13例AML中仅 2例因单纯腹部胀痛就诊 ;左右侧及男女性别患病差别不大 ,B超和CT等确诊率高 ( 10 0 % ) ,瘤体直径大于 3 .5cm者 11例 ,手术 10例 ,穿刺活检 3例。结论 B超、CT等影像学方法是诊断AML的首选方法 ,瘤体直径大于 3 .5cm或症状明显者宜尽早手术治疗。

Objective To explore the clinical features,diagnosis and treatment of adrenal myelolipoma(AML).Methods 13 cases of AML encountered during 1978～1998 were reviewed and analyzed.Results Of 13 patients only 2 complained mild flank discomfort,and the incidence had no significant difference in sex or lateral.The diagnostic accuracy of B ultrasound and CT in these patients were both 100%.In 11 out of 13 paitents, the sizes of tumor were larger than 3.5cm in diameter,of whom ten underwent operation,the others were punctured with fine needle under B ultrasound.Conclusions Radiological examination such as B US or CT is the first chozen study in the evaluation of AML.The operation should be performed as soon as possible if the size of tumor is larger than 3.5cm in diameter or the symptom is severe. 〔
