超声引导下甲状腺针吸细胞学检查在甲状腺疾病诊断中优越性的临床研究

Ultrasound-guided Thyroid Fine-needle Aspiration Cytology Clinical Research Superiority in the Diagnosis of Thyroid Diseases

目的探讨超声引导下甲状腺针吸细胞学检查在甲状腺疾病诊断中优越性。方法对照110例甲状腺疾病穿刺细胞学与临床病理对照的符合率。结果甲状腺炎46例,临床确诊44例,符合率为96%。其中,淋巴细胞性甲状腺炎(桥本氏病)38例,临床确诊36例,符合率为98%。亚急性甲状腺炎8例,临床确诊8例,符合率为100%。Graves甲亢11例,临床确诊11例符合率为100%,桥本甲亢2例,临床确诊2例。符合率为100%,甲状腺乳头状癌[1]8例,病理确诊7例,另外一例未手术,符合率为100%。结节性甲状腺肿48例,临床确诊47例,符合率为97%。剩余3例不能诊断。结论超声引导的甲状腺针吸细胞学诊断确诊率可达95%以上,不仅方法简单,易掌握,而且费用低,痛苦小,易接受,可快速做出诊断。

Objectire Ultrasound guided thyroid fine needle aspiration cytology in the diagnosis of thyroid diseases superiority.M ethods of control in 110 cases with thyroid disease aspiration cytology and pathology of coincidence rate.Results of thyroiditis in 46 cases,clinical diagnosis in 44 cases,with the rate of 96%.Wherein,lymphocytic thyroiditis（Hashimoto＇s disease）in 38 cases,clinical diagnosis in 36 patients,with the rate of 98%.8 cases of subacute thyroiditis,clinical diagnosis in 8 cases,with the rate of 100%.Graves hyperthyroidism in 11 cases,11 cases of clinical diagnosis coincidece rate was 100% in 2 cases with hyperthyroidism,Hashimoto,clinical diagnosis in 2 cases.The coincident rate was 100%,[1]8 in thyroid papillary carcinoma cases,confirmed by pathology in 7 cases,one cases without operation,with the rate of 100%.48 cases of nodular goiter,47 cases of clinical diagnosis,with the rate of 97%.The remaining 3 cases could not be diagnosed.Conclusion B ultrasound guided needle aspiration of the thyroid gland cytology diagnosis rate can reach more than 95%,not only has the advantages of simple method,easy control,and low cost,little pain,easy to accept,can make a quick diagnosis.
