视网膜血管增生性肿瘤的影像学特征 被引量:2

摘要 目的探讨视网膜血管增生性肿瘤的眼底影像学特征。方法回顾我院所诊治的视网膜血管增生性肿瘤5例(5只眼),分析其眼底特征、荧光素眼底血管造影(FFA)、眼部B超、眼底相干光断层扫描(OCT)等影像学特征。结果肿瘤为单发、边界清晰、大小不一,肿瘤呈红色,表面大量血管分布、可见小片状出血灶及数支由视网膜表面延伸而来的大血管,瘤体周围视网膜内及视网膜下大量黄白色渗出、范围较广。FFA可见视网膜血管增生性肿瘤瘤体表面清晰的毛细血管网,血管扩张,荧光素渗漏明显,瘤体附近视网膜血管迂曲扩张,荧光素渗漏。眼部B超可见视网膜前实性回声病灶。OCT显示视网膜神经上皮脱离,神经上皮层内及神经上皮层下可见强回声。结论视网膜血管增生性肿瘤以典型视网膜红色瘤样病变为特征,合并视网膜内及视网膜下大量渗出,B超、FFA、OCT等影像学检查能协助诊断,并有助于研究该病的病理学及病程进展。 Objective To investigate the characteristics of fundus imaging of retinal vascular proliferative tumors. Methods Review of 5 cases (5 eyes) d/agnosed as retinal vascular proliferative tumors, analyzed the chameteristlcs of fundus fluoresce/n angiography (FFA), B ultrasound and retinal optical coherence tomography (OCT) imaging features. Results Tumor is a single, boundary clear mass. its size is different. The tumor was red, the surface was distributed by blood vessels, visible small pieces of hemorrhage and large blood vessels extending from the retinal surface. Subretinal yellow-whlte exudate around the tumor. FFA showed capillary network on the tumor surface. dilated blood vessels and leak- aged fluoresceln, The retinal vascular near the tumor is expanded and fluorescein leakage. B scan showed solid echo. OCT showed retinal neuroepithelium detachment and visible hyperechoic. Conclusion Retinal vascular proliferative tumor char- acterized by typical retinal red tumor-fike lesions and large retinal and subretinal exudation, B scan, FFA, the OCT ima- ging examinations can help diagnoee, and help research the pathology and progression of the retina.
