摘要
目的 :评价99mTc -PPM平面显像诊断肺癌的临床价值。材料与方法 :2 8例经X线、CT证实肺肿瘤患者 ,其中 2 0例恶性 ,8例良性。注射99mTc -PPM后 5min、15min、30min、1h行平面显像 ,选延迟相图像以ROI法处理 ,计算T/NT ;并与99mTc -MIBI比较。结果 :8例良性肿瘤的99mTc -PPMT/NT值为 1.0 4± 0 .0 8;2 0例恶性肿瘤的T/NT值为 1.41± 0 .16。以T/NT值 1.2为标准 ,99mTc -PPM对肺癌诊断的灵敏度、特异性和准确度分别为 90 % (18/2 0 )、87.5 % (7/8)、89.3 % (2 5 /2 8) ;该结果与99mTc -MIBI相比无明显的差异。结论 :99mTc -PPM可用于鉴别肺肿瘤显像 。
Purpose:To evaluate the clinical value of 99m Tc-PPM (Peplomycin) planar imaging in the diagnosis of lung cancer. Materials and methods: 28 patients, suffering from 20 malignants and 8 benign lesions in lungs, were evaluated by chest X-ray and CT scan. Patients were given a 555MBq 99m Tc-PPM. Planar images were taken at 5min、15min、30min、1h and 2h after injection. ROIs were drawn over tumor and control site on control side. Ratio of T/NT was calculated on 30min image. Results: The ratio of T/NT in 8 benign lesions was 1.04±0.08. The ratio in 20 malignancy was 1.41±0.16. The sensitivity, specificity and accuracy of identifying malignant lesion was 90% (18/20)、87.5%(7/8)、89.3%(25/28) respectively for 99m Tc-PPM. There is no difference in diagnostic peformance between 99m Tc-PPM and 99m Tc-MIBI. Conclusion: 99m Tc-PPM is an useful imaging agents in differentiating malignant lung neoplasm from benign lesions.
