儿童无骨折脱位型胸腰髓损伤的临床分析

Clinical Analysis of Thoracolumbar Spinal Cord Injury without Fracture and Dislocation in Children

目的 :通过对 6例儿童无骨折脱位型胸腰段脊髓损伤的病例分析 ,进一步认识本病。方法 :本组 6例全部为胸腰段脊髓损伤。其中不完全脊髓损伤 5例 ,完全性脊髓损伤 1例 ,根据Frankel脊髓损伤分类法 :A级 1例 ,B级 2例 ,C级 3例。保守治疗 5例 ,手术治疗 1例。结果 :经 6个月～ 9年 10个月随访 ,4例完全恢复 ,1例参照Frankal分级法 ,半年后由入院时的B级恢复至E级 ,1例完全性脊髓损伤病儿无恢复。结论 :如果治疗及时、恰当 ,其治疗结果较为满意 ,对于有脊髓损伤而X线片甚至CT未发现脊柱异常者 ,应做MRI检查 ,以免延误治疗时机。

Objective: 6 cases of thoracolumbar spinal cord injury without fracture and dislocation in children were analysed for further research the mechanism and clinical therapy. Method: All 6 cases were thoracolumbar spinal cord injury. The type of spinal cord injury were classified as complete injury in 5, incomplete injury in 1.1 case was grade A, 2 cases were grade B, 3 cases were grade C according to Frankel. Conservative therapy in 5, Operative in 1. Results: The time of followed up from 6 months to 9 years and 10 months. 4 cases recovered completely. 1 case was inproved from grade B to grade E 6 months later. 1 complete injury child was not recovered. Conclusion: The result is satisfied through early correct therapy. Spinal cord injury patients should have MRI examination if their X ray or CT display normally to avoid delaying therapy.
