重比重布比卡因单侧腰麻在高龄患者单侧下肢手术中的疗效

The effectiveness of the hyperbaric bupivacaine used in unilateral spinal anesthesia for the lower - extremity surgery in the advanced age patients

目的评价重比重布比卡因单侧腰麻用于高龄患者单侧下肢手术的疗效。方法高龄患者单侧下肢手术患者80例,随机分为重比重布比卡因单侧腰麻组(0.75%布比卡因和5%葡萄糖2∶1体积混合,A组)和等比重布比卡因单侧腰麻组(0.5%布比卡因,B组),每组40例,速率均为0.1～0.2 mL.s-1,观察2组患者麻醉前后的血压和血流动力学情况。结果 2组患者麻醉效果差异无统计学意义。2组患者麻醉给药后的血压比较,差异有统计学意义(P<0.05);A组患者麻醉给药后血压与麻醉前的血压比较,差异无统计学意义(P>0.05)。结论重比重布比卡因单侧腰麻对患者血流动力学影响小,适合老年高龄患者下肢手术的麻醉。

Objective To evaluate the effectiveness of the hyperbaric bupivacaine used in unilateral spinal anesthesia for the lower extremity surgery in the advanced age patients. Methods Eighty elder patients, undergoing unilateral lower extremity surgery, were randomly divided into two groups, forty cases in each group. Group A received hyperbaric bupivacaine while group B received isobaric bupivacaine for unilateral spinal anesthesia. To explore the hemodynamics changes before and after anesthesia in both groups. Results There was no significant difference of anesthesia effect between the two groups. Compared with group B, group A had significant difference of blood pressure after anesthesia （ P 〈 0.05 ）. In group A, ,there was no significant difference of blood pressure before and after anesthesia （P 〉 0. 05 ）. Conclusion Hyperbaric bupiv acaine is a more suitable drug for elderly patients with lower limbs opera tion because its block segment is limited and has marginal influences on hemodynamic effect.
