手术治疗无骨折脱位型颈脊髓损伤31例

Surgical treatment for no fracture dislocation cervical spinal cord injury

目的探讨无骨折脱位型颈脊髓损伤的外科治疗方法及临床疗效。方法 2009年1月~2011年12月共收治31例无骨折脱位型颈脊髓损伤患者,均行手术治疗,并对治疗患者进行回顾性分析,探讨其手术疗效。结果所有患者均获得6个月~2年随访,平均9.8个月。按美国脊髓损伤学会脊髓功能评定标准,Frankel分级评定神经功能恢复除2例稍差外其余均有明显提高。按JOA评分,术前平均5.24分,术后提高到10.97分。结论无骨折脱位型颈脊髓损伤根据脊髓受压的部位和范围选择不同的手术方式,争取早期手术,可有效地改善脊髓功能,达到较好的治疗效果。

【Objective】To investigate the method and effect of surgical treatment for non fracture-dislocation of cervical spinal cord injury.【Methods】31 patients with non fracture dislocation cervical spinal cord injury were underwent surgical treatment.The clinical examinations were carried out to observe the therapeutic effects throw the follow-up.【Results】The follow-up duration was from 6 months to 2 years,with the average of 9.8 months.Frankel grade assessment of neurological function was significantly improved except two cases.All the cases by JOA estimating,preoperative average of 5.24 points,increased up to 10.97 after operation.【Conclusion】No fracture dislocation cervical spinal cord injury,according to segment parts of the spinal cord compression should early treated by surgical treatment,and selecting the appropriate surgical approach may be better to improve spinal function and the life quality of patients.
