摘要
目的:应用荧光原位杂交(fluorescence in situ hybridization,FISH)技术检测宫颈脱落细胞中hTERC(human telomerase gene)的表达,探讨其临床意义。方法:搜集宫颈脱落细胞104例,其中正常20例,CINⅠ23例,CINⅡ14例,CINⅢ26例,宫颈癌21例。采用FISH检测hTERC基因表达,以正常标本建立阈值,同时进行液基细胞学(TCT)及第二代杂交捕获(HC-Ⅱ)检测。结果:hTERC阈值为5.91%,hTERC扩增:CINⅠ3例(13.04%)、CINⅡ7例(50.00%)、CINⅢ20例(76.92%)、宫颈癌21例(100.00%),各组间比较差异具有统计学意义(χ2=45.66,P<0.001);67例HPV阳性者,hTERC阳性率为65.67%(44/67),37例HPV阴性者,hTERC阳性率为18.92%(7/37),两组比较差异有统计学意义(χ2=7.5,P<0.01),hTERC扩增与HPV感染正相关(r=0.592);hTERC扩增率在HSIL及以上病变中高于LSIL及以下病变(χ2=28.69,P<0.005)。结论:hTERC有可能作为辅助宫颈癌筛查指标之一。
Objective To detect the expression and clinical significance of hTERC in cervical exfoliated cells by fluorescence in situ hybridization (FISH). Methods 104 cases of cervical exfoliated cells were collected, including 20 cases of normal cervical exfoliated cells, 23 cases of CIN Ⅰ , 14 cases of CIN Ⅱ , 26 cases of CIN Ⅲ, and 21 cases of cervical cancer. The expression of hTERC was detected by FISH, and the threshold of hTERC was established with normal samples, meanwhile, TCT and HC- I] were adopted. Results The threshold of hTERC was 5.91%. The positive numbers of hTERC were 3 cases (13.04%), 7 cases (50.00%), 20 cases (76.92%) and 21 cases (100.00%) in CIN Ⅰ , CIN Ⅱ, CINⅢ, and cervical cancer (χ2 = 45.66, P〈 0.001). The hTERC positive rates were 65.67% in 67 HPV positive patients and 18.92% in 37 HPV negative ones (χ2 = 7.5, P 〈 0.01). The amplification of hTERC was positively correlated with HPV infection (r = 0.592). The rates of hTERC in HSIL and above were higher than those of LSIL and the following.
