冠心病不稳定型心绞痛中医辨证分型与hs-CRP的相关性研究

Correlation Between the Levels of hs-CRP and Differentiation of Symptoms and Signs for Classification of Syndrome

目的:探讨不同中医证型冠心病不稳定型心绞痛患者hs-CRP水平,及与冠状动脉狭窄程度的相关性。方法:本研究共收集符合纳入标准的冠心病不稳定型心绞痛患者121例。分为气虚血瘀、痰瘀互阻、阳虚寒凝、心脾两虚及心肾阴虚5种证型。另外选取非心绞痛患者60例作为对照。检测各组hs-CRP水平,并对入选患者进行相应的冠状动脉造影检查,分析hs-CRP与冠状动脉狭窄程度、中医证型之间的相关性。结果:气虚血瘀组、痰瘀互阻组和阳虚寒凝组hs-CRP含量均高于心脾两虚组和心肾阴虚组(P<0.05),而且冠状动脉血管病变损害程度也高于心脾两虚组和心肾阴虚组,且主要以重度狭窄为主(P<0.05);hs-CRP与狭窄程度总积分相关性比较发现hs-CRP与狭窄程度呈正相关(r=0.68752,P=0.01)。结论:气虚血瘀和痰瘀互阻是冠心病不稳定型心绞痛发作期主要证型,hs-CRP的含量与冠状动脉血管狭窄程度呈正相关,是冠心病不稳定型心绞痛预后的预测因子。

Objective ： To explore correlation between the level of hs - CRP and the type of syndrome in unstable angina pectoris （UAP）. Methods ： 121 UAP patients were divided into 5 syndrome groups according to diagnostic criteria：deficiency of vital energy and blood stasis syndrome ;sputum stasis hinder mutually syndrome;yang asthenia and cold accumulation syndrome;cardio- splenic asthenia syndrome ; heart and kidney yin asthenia syndrome. The content of hs - CRP and coronary arteriongraphy were detected for each group, to study the correlation between the level of hs -CRP and the type of syndrome and coronary artery stenosis in UAP. Results ： hs - CRP level of deficiency of vital energy and blood stasis syndrome, sputum stasis hinder mutually syndrome and yang asthenia and cold accumulation syndrome were higher than that of cardiosplenic asthenia syndrome, heart and kidney yin asthenia syndrome （P 〈 0. 05 ）, meanwhile the level of coronary artery stenosis was more severe than that of cardiosplenic asthenia syndrome and heart and kidney yinasthenia（ P 〈 0. 05 ）, and severe stenosis was the major appearance for the three syndromes（ P 〈 0. 05 ）. It is positive correlation between the total integration of stenosis and hs - CRP content（ r = 0. 68752 ,P = 0.01 ）. Conclu- sion ： Deficiency of vital energy and blood stasis syndrome and sputum stasis hinder mutually syndrome were the main type of syn- drome for UAP, and hs - CRP was prediction factor for UAP prognostic.
