小剂量阿司匹林在糖尿病视网膜病变光凝术后的作用

摘要 目的观察糖尿病视网膜病变激光光凝术后使用小剂量阿司匹林的作用。方法 116例(214眼)糖尿病视网膜病变患者随机分为两组,治疗组60例(110眼),全视网膜光凝术后予小剂量阿司匹林(100 mg/d);对照组56例(104眼),只行全视网膜光凝术。比较两组的治疗效果。结果术后1、3个月后,两组视力改善差异无统计学意义(P>0.05);术后6个月,两组视力改善均较术后3个月时下降,但治疗组优于对照组,差异有统计学意义(P<0.05)。术后1个月,两组视网膜新生血管、水肿、渗出、出血改善情况相似,差异无统计学意义(P>0.05);术后3个月,两组均继续好转,但治疗组要明显好于对照组,差异有统计学意义(P<0.05);术后6个月时,两组视网膜病变均出现恶化,但治疗组病变好于对照组,差异有统计学意义(P<0.05)。结论糖尿病视网膜病变光凝术后使用小剂量阿司匹林,可延缓眼底病变恶化,改善患者视力,提高手术疗效。 Objective The study was to observe the effects of low-dosage aspirin on diabetic retinopathy after retinal photocoagulation. Method I16 patients(214 eyes) with diabetic retinopathy were randomly divided into an experiment group(60 patients, 110 eyes), which took low-dosage aspirin (100mg/d) after retinal photocoagulation and a control group (56 patients, 104 eyes) , which were simply treated with photocoagulation therapy. The therapeutic effects on both groups were compared. Results There was no difference in vision improvement between the two groups after one and three postoperative months. After six postoperative months, vision of the two groups decreased, but vision of the experiment group was better than that of the control group. After one postoperative month, the improvement of ocular fundus hemorrhage, edema and exudation were similar between two groups. After three postoperative months, the status of ocular fundus was improved continually, while the improvement of the experiment group was more obvious than that of the control group. The status of ocular fundus at six months deteriorated in both groups, but the status of therapeutic group was better. Conclusion Low-dosage aspirin on diabetic retinopathy after retina| photocoagulation may delay deterioration of the status of ocular fundus, improve patients' vision and enhance the effect of retinal photocoagulation.
