食管癌术后早期目标容量导向治疗对心血管系统的影响

Early Goal Directed Therapy Capacity Effects on the Cardiovascular System after Esophageal Cancer Operation

目的探讨食管癌术后早期患者采用目标容量导向治疗对心血管系统的影响.方法将100例胸段食管癌患者随机分2组,每组50例,采用相同术前准备、手术方式,术后观察组采用目标容量导向治疗,对照组采用传统液体治疗方法,比较术后2组不同时间的心率(RT)、收缩压(SBP)、术后液体平衡量Fb(mL)、心脏并发症.结果术后出现心律失常、心力衰竭的患者观察组均优于对照组,术后1～4 d观察组心率增加值均低于对照组,有统计学意义(P<0.01);术后第5天差别无统计学意义(P>0.05).术后1～2 d观察组液体正平衡量少于对照组,有统计学意义(P<0.01);术后3～4 d观察组液体负平衡量少于对照组,有统计学意义(P<0.01);术后第5天液体平衡差别无统计学意义(P>0.05).结论采用术后早期目标容量导向治疗可以改善心血管系统功能,减少并发症的发生.

Objective To explore the early postoperative patients with esophageal carcinoma by target capacity oriented therapy effects on the cardiovascular system. Method 100 cases with thoracic esophageal cancer were randomly divided into 2 groups,50 cases in each group,using the same preoperative preparation, operation way, postoperative observation group using target capacity oriented therapy, the control group using the traditional liquid treatment method, comparison of different times after the operation of heart rate （ RT ）, systolic blood pressure （ SBP）, postoperative liquid equilibrium of Fb （ mL）, cardiac complications. Results The postoperative arrhythmia, heart failure in observation group are better than those of control group, increased values of heart rate after 1-4 d operation in observation group were lower than that of the control group, there was statistically significant （ P 〈 0.01 ） ; there had no significant difference （ P 〉 0.05 ） after 5 d ; positive fluid balance ofobservation group is less than that of the control group after 1-2 d, there is statistical significance （P 〈 O. 01 ） ;the negative fluid balance in observation group is less than that of the control group after 3-4 d operation,there was statistically significant difference （P 〈 0.01 ） ; the fluid balance after 5 d operation had no statistical significance （P 〉 0.05 ）. Conclusion The early postoperative target capacity oriented therapy can improve the function of cardiovascular system and reduce complications.
