子宫内膜异位症术后复发的相关因素分析

Relative factors of endometriosis postoperative recurrence

目的探讨分析子宫内膜异位症(EM)术后复发的相关因素,从而指导个体化治疗。方法回顾性分析2008年1月至2011年1月期间在我院妇科收治的140例EM患者的临床资料。所有患者均行手术治疗,术后随访两年,观察患者的年龄、术中与术后美国生育学会制定的EM分期标准(R-AFS评分)、手术治疗方式以及术后药物治疗4项指标,同时随访记录复发情况和复发时间,将所得数据进行统计学分析,从而分析EM术后复发的相关因素。结果随访两年共有40例患者复发,第1年22例复发,复发率为15.7%,第2年累计复发率为28.6%。统计学分析后显示:患者处于生育年龄、R-AFS评分偏高、保守性手术治疗方式及术后未用药治疗是危险性相关因素;而患者年龄大于40岁、R-AFS评分偏低、根治性手术治疗方式及术后给予孕激素治疗6个月是保护性相关因素。结论 EM术后复发的相关因素有:患者年龄、EM分期、手术治疗方式以及术后是否用药治疗。

Objective To study the relative factors of endometriosis （EM） postoperative recurrence and give individual therapy guidance. Methods Retrospectively analyzed 140 cases with EM from Jan 2008 to Jan 2011 in our hospital. All of the patients were followed up for two years after surgery. The patient＇s age, intraoperative and postoperative R-AFS rating, surgical therapy treatment and postoperative medication were observed. At the same time, recurrence and recurrence time were followed up and recorded. Then made statistical analysis and analyzed the relative factors of EM. Results 40 patients relapsed. The recurrence rate was 15.7% after one year; the recurrence rate was 28.6% after two years. The risk factors were patients at child- bearing age, high R-AFS score, the conservative surgical treatment way and no postoperative medicine; protective related factors were patients older than 40 years old, low R-AFS score, radical surgery treatment and progestogen treatment for 6 months after the surgery. Conclusion The relative factors of EM are patienti＇s age, EM stage, surgical treatment way and whether the drug treatment.
