彩超引导下颌面颈部肿块粗针穿刺活检的临床价值

The Clinical Value of Color Doppler Ultrasound-Guided Core Needle Biopsy in Diagnosing Maxillofacial Neck Mass

目的探讨彩色超声引导下穿刺活检在颌面颈部肿块诊断中的作用,评价其有效性及并发症。方法对颌面颈部肿块术前需明确诊断而难以常规活检的病例,在彩色超声引导下,用美国产Bard Magnum活检枪和配套18G活检针经皮穿刺活检,标本送组织病理学检查,并与术后病理结果相比较。结果穿刺7例,均取材成功,6例可明确诊断,恶性肿瘤3例,良性肿瘤2例,炎症1例。涎腺组织来源3例,淋巴组织来源2例,神经组织来源1例。6例行手术治疗,术后病理均与术前穿刺活检诊断相符合。无一例出现大出血及其他严重并发症。结论超声引导穿刺活检术诊断准确率高,操作简单、并发症少,在颌面颈部肿块的诊断中具有重要的临床应用价值,可作为首选检查方法。

Objective To investigate the function of ultrasound-guided needle biopsy in the diagnosis of maxillofacial neck mass. Methods The percutaneous punctures biopsies under ultrasound guidance were performed for the patients who suffered maxillofacial neck mass that can not be diagnosed through conventional biopsy methods. The obtained samples were sent for pathological examinations. The diagnosis was compared with that after operations. Results All of the samples were obtained successfully in a total of 7 patients. 6 patients can be diagnosed definitely. 3 samples were malignant tumors. 2 samples were benign tumors. 1 was diagnosed as inflammation. 3 samples were originated from sali- vary glands. 2 samples were originated from lymphatic tissue. 1 sample was originated from neural tissue. 6 pathological diagnosis after operation were same as that before operation. No case of bleeding and other serious complications. Con- clusion Ultrasound-guided needle biopsy was a good sampling method with＇high sampling successful rate,operating fa- cility,high histological diagnostic accuracy and low ratio of complication. It holds significant clinical value in the diagno- sis of maxillofacial neck mass.
