改良式B-lynch缝合术在终止中期子宫疤痕处妊娠的临床应用

A Clinical Study on Modified B-lynch Suture Technique in Terminating Interim Uterine Scar Pregnancy

目的探讨改良式B-lynch缝合术在终止剖宫产术后中期子宫疤痕处妊娠术中出血的止血效果。方法在我院2008～2011年收治剖宫产术后中期子宫疤痕处妊娠的临床资料中,B超诊断中央型胎盘前置状态,部分胎盘均附着在子宫前壁原手术瘢痕处,在行剖宫取胎术中因术中出血超过500ml的病例中采用了改良式B-lynch缝合术的病例共计10例进行了回顾性分析。结果 B-lynch缝合术在中期子宫疤痕处妊娠剖宫取胎术中止血的疗效显著。结论改良式B-lynch缝合术是终止剖宫产术后中期子宫疤痕处止血的有效措施,适合在基层医院推广使用。

Objective To discuss the intraoperative bleeding hemostatic effect of modified B- lynch suture technique in terminating interim uterine scar pregnancy. Methods The clinical data on uterine scar pregnancy after cesarean section in our hospital from 2008 to 2011 showed that in the precondition of central type placenta diagnosed by B ultrasonic, some placentae were at- tached to the uterine wall of operation scar. Then analyzed were the 10 cases which bleeded for more than 500 ml in cesarean and were treated with modified B-lynch suture technique.Results Ten cases of the modified B-Lynch suture postoperative bleeding are effective and successful retained the uterus and the prognosis is good. In the follow-up of 1-2 month menstruation when all returned to normal, B ultrasonic uterine and accessories showed no abnormalities, the two cases of pregnancy in again. Conclusion Being simple and effective in homeostasis, it is worthwhile to adopt B-lynch suture technique in the middle of uterine scar preqnancv.
