十二指肠疾病临床及内镜特点分析

Clinical types and endoscopic presentations for duodenal diseases

目的:探讨十二指肠疾病发病情况及内镜表现。方法:回顾总结分析我院十年来住院患者中确诊的十二指肠疾病病例的临床特点和内镜下特性。结果:2406例十二指肠疾病患者中,男性1487例,女性919例。其中十二指肠溃疡1320例,十二指肠息肉831例,十二指肠恶性肿瘤172例,十二指肠憩室30例,十二指肠良性肿瘤19例,先天性巨十二指肠10例,十二指肠球炎10例,十二指肠瘘8例,十二指肠钩虫病2例,十二指肠克罗恩病2例,十二指肠囊肿1例,十二指肠先天性闭锁1例。十二指肠疾病疾的病谱较以往文献报道要广,且男性发病率明显高于女性,但十二指肠恶性肿瘤男女发病无明显区别。结论:十二指肠疾病的病灶分布较广,但溃疡发生率相对较高。CT、胃镜、十二指肠镜联合超声内镜检查可诊断十二指肠疾病。

Objective： To investigate the clinical types of duodenal dis- ease and its presentations under endoscopy. Methods ： The clinical features and endoscopic findings were retrospectively an＇alyzed in inpatients with duodenal lesions undergone treatment in our hospital between Jan. 2002 and 2012. Results ： Of the total 2 406 patients, 1 487 were male and 919, female. Duodenal ulcer was seen in 1 320 cases, polyp of duodenum in 831 ,malignant neoplasm of duodenum in 172,duodenal diverticulum in 30,benign neoplasm of duodenum in 19, congenital giant duodenum in 10, inflammation of duodenal in 10, duodenal fistula in 8, ancylostomiasis duodenale in 2 ,Crohn＇s dieases of the duodenum in 2 ,duodenal cyst in 1 and congenital atresia of duodenum in 1. The lesions of duodenal disease were found more involvement in ours than previous reports. Although the males were more easily affected than females, yet they had no significant difference concerning progression to malignancy. Conclusion：Duodenum has a wider lesion involvement, and duodenal ulcer appears the most prev- alent. Combined detection with CT, conventional endoscopy and endoscop- ic uhrasonography may generally make differential and final diagnosis of duodenal disease.
