椎动脉颅内段:一个被忽视的区域 被引量:2

摘要 在后循环所有的动脉中,我们对椎动脉颅内段关注最少,特别是在闭塞病变的治疗方面。早期的临床病理研究表明,在延髓外侧及小脑后下动脉分布区梗死的患者中经常发现椎动脉颅内段闭塞。新英格兰医学中心后循环登记(the New England Medical Center Posterior Circulation Registry,NEMC-PCR)408例患者中,严重的椎动脉颅内段狭窄或闭塞最为常见,其中椎动脉远端最易受累,有时还累及基底动脉。椎动脉颅内段闭塞常累及双侧,并且常伴有基底动脉及颈部椎动脉闭塞。双侧椎动脉颅内段闭塞患者症状有突发眩晕/视物模糊及共济失调等,病程迁延数月或者数年,但很少会发展成致残性卒中。大多数椎动脉颅内段闭塞患者的梗死灶多位于后循环供血区的中央及远端,吻侧可以到达延髓及小脑下部。虽然目前的诊断技术可以很好地显示椎动脉颅内段及梗死部位,但对不同的椎动脉颅内段闭塞患者的最佳治疗方案却少有研究报道。椎动脉颅内段疾病的治疗仍是一个未知的领域,亟需关注及解决。
