IgA肾病患者尿NGAL和免疫球蛋白轻链的水平及临床意义 被引量:4

摘要 目的探讨IgA肾病患者尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和免疫球蛋白轻链(kapU、lamU)水平及其与临床病理指标的相关性。方法检测54例IgA肾病患者的NGAL、kapU和lamU水平并分析其与常见病理指标的相关性。结果 (1)IgA肾病组的尿NGAL、LamU和kapU均高于健康对照组(P<0.01),在Lee氏分级、肾小管间质损伤及Katafuchi总积分的不同程度上有统计学差异;(2)IgA肾病组的尿NGAL、LamU和kapU均与Scr、24h尿蛋白、血IgA、sAlb、Katafuchi总积分、尿β2-微球蛋白、BUN呈正相关,与eGFR、尿渗透压呈负相关,且均有统计学差异;(3)NGAL≤100μg/L,lamU≤20×10-3g/L和KapU≤20×10-3g/L的显效率高于对应组(P<0.01)。结论 IgA肾病患者的尿NGAL、kapU和lamU水平可用于预测治疗效果。
