腹腔镜完全腹膜外腹股沟疝修补术的临床体会(附85例报告)

Clinical experience of totally extraperitoneal laparoscopic repair of inguinal hernia: with a report of 85 cases

目的:探讨腹腔镜完全腹膜外腹股沟疝修补术(totally extraperitoneal prosthesis,TEP)的可行性,并评价术中、术后并发症、复发率等临床效果。方法:回顾分析2012年1月至2012年6月全麻下为85例(96侧)患者行腹腔镜TEP的临床资料。结果:84例(95侧)手术获得成功,1例(1侧)中转开腹。1例术后发生远端疝囊血清肿,经穿刺抽吸治疗后好转,并发症发生率为1.2%。术后平均随访(7±0.8)个月,无复发,患者无明显不适感。结论:腹腔镜TEP安全、可行,并发症发生率低,复发率低,优势明显。

Objective： To explore the feasibility of totally extraperitoneal laparoscopic inguinal hernia repair, and evaluate its effect with regard to intraoperative and postoperative complications as well as recurrence rate. Methods：A retrospective analysis was made on the clinical data of 85 patients （96 sides）, who underwent laparoscopically totally extraperitoneal prosthesis （TEP） under general anesthesia between Jan. 2012 and Jun. 2012. Results ： Eighty-four procedures （95 sides ） were performed successfully, while one case （1 side） was converted to open surgery＇. Distal hernia sac seroma was observed in one patient （1.0%） , and was relieved after needle aspiration. The mean postoperative follow-up was （7 ± 0.8 ） months. The recurrence was not observed. None of the follow-up patients complained late discomfort. Conclusions ： Laparoscopic TEP for inguinal hernia is safe and feasible with obvious advantages, such as less complications and lower recurrence rate.
