影响功能性鼻内镜手术治疗真菌性鼻及鼻窦炎预后的危险因素分析及预防对策

Study on the risk factors and preventive countermeasures of functional endoscopic sinus surgery in the treatment of fungal nasal sinusitis

目的探讨影响功能性鼻内镜手术治疗真菌性鼻及鼻窦炎预后的危险因素，并采用相对应的措施予以预防，以便提高临床效果。方法选取湖北省阳新县人民医院2010年10月～2012年12月运用鼻内镜手术治疗真菌性鼻及鼻窦炎110例患者，分析影响其预后不佳的危险因素。结果影响功能性鼻内镜手术治疗真菌性鼻及鼻窦炎预后的危险因素有工作环境在室外、手术范围广、有多种合并症、农村患者、存在有鼻息肉、术后未综合治疗（P〈0．05），而与患者的年龄、性别、文化程度、病程、病变范围、主治医生职称和是否长期用药无关（P〉0．05）；其中工作环境、多种合并症、综合治疗是影响功能性鼻内镜手术治疗真菌性鼻及鼻窦炎预后的高危险因素（P=0．025、0．003、0．031）。结论鼻内镜手术治疗真菌性鼻及鼻窦炎是目前有效的方法，但是工作环境的区别、多种合并症、是否综合治疗对其效果影响性大。

Objective To study the risk factors on functional endoscopic sinus surgery in the treatment of fungal nasal sinusitis, and the corresponding measures are took to prevent and improve the clinical effect. Methods 100 patients with fungal nasal sinusitis who treated by nasal endoscopic surgery in the People＇s hospital of Yangxin county from October 2010 to December 2012 were selected, and the risk factors were analyzed. Results the risk factors which in- fluenced functional endoscopic sinus surgery in the treatment of fungal nasal sinusitis were work environment in out- door, extensive operation, a lot of complications, rural patients, postoperative nasal polyp, and no comprehensive treatment after operation （P 〈 0.05）. There was no relationship on the patient＇s age, gender, educational level, course of disease, lesions scope, attending physician titles and long-term medication （/9 〉 0.05）; the working environment, a variety of complications, comprehensive treatment were high risk factors in functional nasal endoscopic surgery in the treatment of noninvasive fungal sinusitis nose and the prognosis of （P = 0.025, 0.003, 0.031）. Conclusion Nasal endoscopic surgery in the treatment of noninvasive fungal sinusitis is an effective method, which has a big effect on the difference between the working environments, a variety of complications, whether in the treatment.
