乳腺癌改良根治术后逆向调强放疗摆位误差的剂量学研究 被引量:3

摘要 目的:探讨乳腺癌改良根治术后逆向调强放疗的摆位误差对靶区内剂量的影响,为临床靶区勾画和计划设计提供参考。方法:采用电子射野影像系统对30例乳腺癌改良根治术后需要行放疗的患者进行摆位误差的测定。每周2次将治疗过程中的电子射野影像系统所拍摄的正位和侧位片,与数字化重建图像的验证片进行对比分析并计算误差。比较初次逆向调强放疗计划(对照组)和参考误差后制定的放疗计划(试验组)二者靶区剂量体积直方图差异。结果:经过290幅图像对比发现,身体前后方向(Z轴)的误差平均为(2.44±2.17)mm;头脚方向(Y轴)平均为(2.01±1.32)mm;左右方向(X轴)平均为(1.27±0.88)mm。对照组和试验组计划靶区体积的V95%分别为98.8%、98.1%;患侧肺的V20分别为28.4%、29.6%;平均剂量分别为1 442.3 cGy、1 490.2 cGy。心脏的V30分别为6.7%、7.1%;平均剂量分别为802.6 cGy、833.5 cGy。结论:乳腺癌改良根治术后采用调强放疗的摆位误差造成的靶区覆盖率和正常组织剂量变化,在临床可接受范围。
