磺胺类药物常见的不合理用药处方分析 被引量:2

摘要 重点提示根据药代动力学特点和临床用途,磺胺类药物可分为:①口服易吸收可全身应用者,如磺胺甲嚷唑、磺胺嘧啶、复方磺胺甲唣唑等;②口服不易吸收者,如柳氮磺吡啶;③局部应用者,如磺胺嘧啶银等。本类药物引起的过敏反应多见,并可表现为严重的渗出性多形红斑、中毒性表皮坏死松解型药疹等,因此过敏体质的患者应尽量避免使用本类药物,禁用于对任何一种磺胺类药物过敏以及对呋塞米、噻嗪类利尿药、磺脲类、碳酸酐酶抑制剂过敏的患者。本类药物可致粒细胞减少、血小板减少及再生障碍性贫血,用药期间应定期检查血常规。本类药物可致肝脏损害,可引起黄疸、肝功能减退,严重者可发生肝坏死,用药期间需定期测定肝功能,肝病患者应避免使用。,本类药物可致肾损害,用药期间应监测肾功能,肾功能减退、失水、休克及老年患者应用本类药物易加重或出现肾损害,应避免使用。本类药物可引起核黄疸,因此禁用于新生儿及2月龄以下婴儿。妊娠期、哺乳期患者应避免使用本类药物。用药期间应多饮水,保持充分尿量,以防结晶尿的发生;必要时可服用碱化尿液的药物。
