海洋胶原肽对高血压动脉粥样硬化、心脏结构和心脏功能的干预作用 被引量:3

摘要 目的观察海洋胶原肽对高血压患者动脉粥样硬化、心脏结构和心脏功能的干预效果并探讨其可能的作用机制。方法选取原发性高血压患者90例随机分为干预组和对照组,在给予低盐低脂饮食和适量运动指导以及常规药物治疗的基础上,同时进食海洋胶原肽或安慰剂(玉米淀粉)作为干预。分别于干预前、干预后1.5、3个月后使用彩色多普勒超声诊断仪检测颈总动脉内径、内中膜厚度(IMT)、斑块情况等颈动脉粥样硬化指标以及主动脉(AO)内径、左心房(LA)收缩期内径、室间隔厚度(IVST)等心脏结构指标和二尖瓣口舒张早期血流速度峰值(E)与舒张晚期血流速度峰值(A)的比值(E/A)、左心室短轴缩短率(FS)和射血分数(EF)等指标。结果总体水平比较,除了干预组的心脏收缩功能指标中干预后1.5、3个月的左心室短轴缩短率(FS)和射血分数(EF)与干预前比较测值增高(P<0.01),与对照组比较有显著性差异(P<0.01),且干预后3个月EF、FS分别与干预后1.5个月比较测值增高,但无显著性差异(P>0.05)以外,其他各组各时相干预前后的颈动脉粥样硬化指标、心脏结构指标比较,均无显著性差异(P>0.05)。结论海洋胶原肽短期内干预对高血压患者颈动脉粥样硬化、心脏结构无明显改善作用,但能在一定程度上改善心脏收缩功能。 Objective To observe the intervention effect of marine collagen peptide on atherosclerosis, cardiac structure and heart function of patients with hypertension and to explore its possible mechanism. Methods 90 hypertension patients were selected and randomly divided into intervention and control groups, based on the given low salt low fat diet and exercise guidance and the routine drug therapy, the cases were given marine collagen peptide or placebo (starch) as the intervention. Before and after intervention respectively 1.5, 3 months, all subjects were detected carotid artery diameter by color Doppler ultrasonic diagnostic apparatus, such as intima-media thickness (IMT), plaque, carotid atherosclerosis index as well as the aorta ( AO), left atrial diameter ( LA), systolic diameter interventricular septal thickness (IVST) and other cardiac structure indexes and mitral early diastolic peak flow velocity (E) and peak late diastolic flow velocity (A). The ratio (E/A), left ventricular fractional shortening (FS) and ejection fraction (EF). Results 18 of 45 cases of intervention group had im-provement effect on carotid atherosclerosis indexes and cardiac function. Overall, in addition to the cardiac systolic function index of left ven- tricular FS and EF measured value were increased timely in intervention group before intervention than those after intervention 1.5,3 months ( P 〈 0. 01 ) . And there were significanf difference compared with those of control group ( P 〈 0. 01 ). The other marks of cardiac structure in- dex and carotid atherosclerosis index during the period of observation in two groups had no significant difference ( P 〉 0.05 ). Conclusions Marine collagen peptide short-term intervention has no effect on carotid atheroselerosis and cardiac structure in patients with hypertension, but could improve cardiac systolic function in a certain extent.
