原发性输尿管息肉67例诊治体会

The experience of diagnosis and treatment of primary ureteral polyps： a report of 67 cases

目的总结原发性输尿管息肉的诊断与治疗体会。方法67例（共69侧）原发性输尿管息肉患者，其中19例行盂管成形，20例输尿管切开息肉切除，15例输尿管镜下息肉电灼或基底部钳除，5例行端端吻合，4例行输尿管下段切除加输尿管膀胱再植，6例因患侧肾重度积水无功能行肾切除。结果术后均经病理证实为原发性输尿管息肉。术后随访0．5～2．5年，均未发现息肉复发与恶变，未发现输尿管狭窄，保留肾脏的病例患肾积水明显缓解。结论原发性输尿管息肉早期诊断比较困难，检查方法可选用超声、影像、输尿管镜等，一旦明确诊断，及时手术。

Objective To discuss the diagnosis and treatment of ureteral polyps. Methods The clinical data of 67 cases of primary ureteral polyps, Nineteen cases were treated with pyeloureteroplasty,20 cases with polypectomy,15 cases with basement fulguration by ureteroscopy operation or polyps-removing by ureter forceps ,5 cases with termino-terminal anastomosis ,4 cases with distal ureter resection and bilat- eral ureterie reimplantation and 6 cases with nephrectomy due to nonfunctioning kidney with severe hydro- nephrosis. Results All cases were confirmed by pathology. All cases had been followed up for 0. 5 ~ 2.5 years with no recurrence, eanceration or stenosis of the ureter, and hydronephrosis was relieved obviously in patients who preserved the kidney. Conclusion It is difficult to make an early diagnosis of primary ureter- al polyps. Once it is diagnosed by ultrasound, imageological examination or ureteroscopy, prompt surgical treatment is essential.
