摘要
目的:探讨鳞状上皮细胞癌抗原(SCC—Ag)检测在预测广西早期食管癌淋巴结转移中的意义。方法:用酶联免疫(enzyme—linkedimmunosorbentassay,ELISA)法检测100例I—II期食管癌患者手术前血清中SCC—Ag水平,行手术确定淋巴结转移情况,比较不同淋巴结转移情况的SCC—Ag水平变化,并进行Logistic回归分析。结果:早期食管癌有淋巴结转移组SCC—Ag水平高于无淋巴结转移组,差异有统计学意义(p〈0.000),经Logistic回归分析,肿瘤大小、浸润深度和SCC—Ag水平是影响早期食管癌淋巴结转移的独立危险因素,应用1KOC曲线(receiveroperatingcharacteristiccurve)分析,SCC—Ag曲线下面积AUC(areundercurve)为0.748,P=0.000,95%CI:0.636-0.860,当SCC—Ag截断值为3.68ng/ml时,预测价值最佳。结论:SCC—Ag水平变化在预测早期食管癌淋巴结转移中有亏要价值.
Objective : To investigate the Significance of squamous cell carcinoma antigen in predicting lymph node metastasis of early stage esophageal cancer. Methods : Serum SCC-Ag level was detected with enzyme-linked immunosorbent (ELISA)in 100 cases I-II patients with esophageal cancer surgery, underwent surgery to determine lymph node metastasis, the SCC-Ag level changes of different lymph node metastasis was compared and Logistic Regression analysis. Results .. Early esophageal cancer lymph node metastasis SCC-Ag level higher than that without lymph node metastasis, and the difference was statistically significant (p 〈0.000), Logistic regression analysis after the show, the tumor size, depth of invasion and SCC-Ag level is an independent risk factor of early lymph node metastasis, Application of ROC curve (receiver operating characteristic curve) was analyzed, the area under the SCC-Ag curve AUC (are under curve) was 0.748, P = 0.000,95% CI :0.636-0 .860, the predictive value of the best When the cutoff value of SCC-Ag was 3.68ng/ml. Conclusions : SCC-Ag level changes has important value in predicting early lymph node metastasis.
