摘要
目的评价舒芬太尼后处理对缺血再灌注大鼠肌钙蛋白Ⅰ的影响,从而探讨其保护作用。方法健康雄性SD大鼠72只随机分为6组:假手术组(Sham组),缺血再灌注组(I/R组),缺血后处理组(Post组),舒芬太尼后处理组(LS组,MS组,HS组)。于缺血前即刻(T0,基础值)、缺血30 min(T1)、再灌注30 min(T2)、60 min(T3)、90 min(T4)和120 min(T5)时记录心率(HR)和平均动脉压(MAP);再灌注120 min末,随机取6只按照ELISA法测定心肌肌钙蛋白Ⅰ(cTnI)并计算心肌梗死面积(IS/AAR),另外6只进行光镜及电镜细胞形态学观察。结果 HR在各组各个时点比较差异无统计学意义(P>0.05),与Sham组比较,I/R组在T3、T4和T5时MAP均降低(P<0.05),而LS组在T3和T4时均降低(P<0.05);与I/R组相比,Post组、MS组和HS组血清中的cTnI、心肌梗死面积及心肌细胞损伤程度显著降低(P<0.05)。结论舒芬太尼后处理能够减轻在体大鼠心肌缺血再灌注损伤血清中cTnI,从而产生保护作用。
Objective To investigate the effect of sufentanil postconditioning on cardiac troponin I induced by myocardial ischemia-reperfu- sion injury in rats and cardioprotection. Methods Male Sprague-Dawley rats [body weight 250 (32 g) ] were randomly divided into six groups ( n =72 ) : Sham (n = 12 ) : the coronary suture was passed, not tied, and maintained for 150 min ; I/R ( n = 12 ) : ischemia for 30 min fol- lowed by 120 min reperfusion;Post(n =12):three cycles of ischemlc postconditioning (30 s of ischemia and 30s of reperfusion) at the onset of reperfusion following 30 rain of ischemia;Sufen (LS,MS,HS,n =12 respectively):respectively administration of 0.75,3 and 15 p.g'kg-1 sufentanil for 3 rain at the onset of reperfusion following 30 min of ischemia. Heart rate and mean arterial pressures were measured at the fol- lowing time points : end of stabilization prior to ischemia (baseline) ; 30 min of ischemia; 30,60,90, and 120 min of reperfusion ; at the end of 120 min reperfusion,the rats were sacrificed for assessment of cTnI,infarct area,cell morphology of light and electron microscopy. Results No significant differences existed in heart rate between groups in all time points (P〉 0.05),mean arterial pressures were significantly de- creased at 60,90,and 120 min following reperfusion (P 〈 0.05) in I/R and at 90 and 120 min following repeffusion (P 〈 0.05) in the LS group compared to Sham;cTnI and infarct area in Post, MS and HS group were significantly decreased compared to I/R group;in cell mor- phology of light and electron microscopy, the damage of myocardial structure was significantly reduced in Post, MS and HS group. Conclusion Sufentanil postconditioning could induce cTnI and protect myocardium against I/R injury.
