声学造影剂及二次谐波成像在肝癌诊断中的作用及局限性 被引量:8

摘要 目的探讨微气泡型声学造影剂和二次谐波成像在肝癌诊断中的作用及局限性。方法13例肝癌患者经外周静脉注射造影剂(剂量0.5-1.0m1)后检查,二次谐波成像用1.7/3.4MHz(ATL—HD15000)或1.8/3.6MHz(HP5500)。结果肝动脉期肝脏多数无明显增强,少数病灶内出现斑点状回声增强,门静脉增强后肝组织才明显增强,但肿瘤组织增强不明显。增强后有利于低回声病灶的显示而对高回声病灶或中高混杂回声病灶显示无益。增强后可显示更多的瘤血管。结论微气泡型声学造影剂及二次谐波成像反映的主要是肝组织总体供血量的变化,相对于正常肝组织来说肝癌总是少血管的,谐波成像可以显示更多的血管。 Objective To investigate the role and limitations of microbubble ultrasound contrast agents and second harmonic imaging in the diagnosis of liver cancer. Methods Thirteen cases of liver cancer patients recieved peripheral intravenous injection of contrast agent( dose of 0.5 - 1.0 ml) after the inspection, second harmonic imaging with 1.7/3.4 MHz ( ATL-HD 15000 ) , or 1.8/3.6 MHz ( HP 5500). Results Most of the hepatic artery of the liver significantly enhanced, punctate echogenic small number of lesions, portal vein enhanced liver tissue significantly enhanced the tumor tissue enhancement was not obvious. Enhanced conducive to the display of the hypoechoic lesions and hyperechoic lesions or mixed echo lesions showed no good. Enhanced to display more of the aneurysm. Conclusions Micro- bubble ultrasound contrast agents and second harmonic imaging reflects the main changes in the overall blood supply of liver tissue relative, to normal liver tissue, liver cancer is always less vascular, harmonic imaging can show more blood vessels.
