外周血Angiopoietin-2 mRNA定量表达与早期胃癌淋巴结转移状态的关系 被引量:3

Correlation Beteeen Quantitative Expression of Angiopoietin-2 mRNA in Peripheral Blood and Lymph Node Metastasis Status in Gastric Cancer

摘要 目的检测早期胃癌患者术前外周血血管生成素-2(Angiopoietin-2,Ang-2)mRNA的定量表达,探讨其与淋巴结转移状态的关系。方法应用实时荧光定量RT-PCR检测62例早期胃癌患者术前和30位健康者外周血Ang-2 mRNA的表达量,以淋巴结组织HE染色和CK20表达检测评价淋巴结转移状态。分析术前外周血Ang-2 mRNA表达量与早期胃癌病理特征、淋巴结转移状态的关系。结果早期胃癌组术前外周血Ang-2 mRNA表达量显著高于正常对照组(3.76±3.15 vs 0.35±0.22,P<0.01)。淋巴结转移阳性胃癌组和淋巴结微转移阳性组外周血Ang-2 mRNA表达量分别显著高于淋巴结阴性胃癌组和淋巴结微转移阴性组(6.84±1.38 vs 3.10±2.21,P<0.01);(4.33±2.16vs 2.82±1.78,P<0.05)。术前外周血Ang-2 mRNA表达量与早期胃癌分化程度、肿瘤大小、浸涧深度均相关(P<0.01),而与肿瘤组织类型无关。单因素分析显示术前外周血Ang-2 mRNA的高表达量与早期胃癌淋巴结转移及淋巴结微转移呈明显相关性(P<0.05)。Logistic多元回归分析显示,术前外周血Ang-2 mRNA表达量是影响早期胃癌淋巴结转移的独立高危因素。结论术前外周血Ang-2 mRNA表达量可能成为评价早期胃癌淋巴结转移的有价值的临床预测指标,有助于指导治疗。 Objective To study the correlation between preoperative quantitative expression of Angiopoietin-2 mRNA in peripheral blood and lymph node metastasis in early gastric cancer(EGC).Methods The preoperative expression levels of Ang-2 mRNA in peripheral blood in 62 patients with EGC and 30 normal controls were measured by real-time quantitaive RT-PCR.The metastasis of lymph node in these cases were determined by HE staining and CK20 detection with immunohistochemistry.The relation between preoperative expression levels of Ang-2 mRNA in peripheral blood and pathologic characterization and lymph node metastasis in EGC was investigated.Results Comparing with normal controls.The experession levels of Ang-2 mRNA in peripheral blood in EGC group were significantly elevated(3.76±3.15 vs 0.35±0.22,P&lt;0.01).The expression levels of Ang-2 mRNA in peripheral blood in the patients with node-positive gastric cancer and in the patients with node micrometastasis gastric cancer were significantly higher than that in the patients with node-negative gastric cancer and in the patients without node micrometastasis gastric cancer respectively(6.84±1.38 vs 3.10±2.21,P&lt;0.01;4.33±2.16 vs 2.82±1.78,P&lt;0.05).The expression levels of Ang-2 mRNA in peripheral blood in the EGC group presented marked correlation with the differentiated degree of the tumor tissue,tumor size,depth of tumor invasion but no correlation with the histological type.Univariate analysis showed that preoperative high expression levels of Ang-2 mRNA in peripheral blood correlated positively with lymph node metastasis and lymph node mierometastasis in ECG(P&lt;0.05).Preoperative expression levels of Ang-2 mRNA in peripheral blood were independent high-risk factor of lymph node metastasis in EGC according to multivariate logistic regression analysis.Conclusion Preoperative expression levels of Ang-2 mRNA in peripheral blood may be a valuable clinical predictor for lymph node metastasis of EGC,which is useful for the treatment direction.
