无创正压通气联合纳洛酮、氨茶碱治疗Ⅱ型呼吸衰竭并发肺性脑病的疗效观察

Clinical observation on treating type Ⅱ respiratory failure with pulmonary encephalopathy by naloxone combined with noninvasive positive ventilation

目的探讨无创正压通气联合纳洛酮、氨茶碱治疗Ⅱ型呼吸衰竭并发肺性脑病的临床治疗效果。方法选择我院70例Ⅱ型呼吸衰竭并发肺性脑病患者,将其分为对照组和观察组各35例;对照组行常规抗炎、止咳祛痰、保持呼吸道通畅等治疗;观察组在对照组基础上给予无创正压通气联合纳洛酮、氨茶碱进行治疗。并于通气前、通气后、治疗结束时对患者的症状、生命体征及通气变化作出相关对比。结果观察组患者总有效率明显优于对照组,动脉血氧分压(PaO2)显著高于对照组,且动脉血二氧化碳分压(PaCO2)显著低于对照组,两组比较差异均有统计学意义(P<0.05)。结论无创正压通气联合纳洛酮、氨茶碱对Ⅱ型呼吸衰竭并发肺性脑病的治疗效果理想,临床治疗较安全,值得在临床中进一步推广应用。

Objective To investigate the efficacy of naloxone combined with non-invasive positive pressure ventila- tion （NIPPV）in the treatment of type Ⅱ respiratory failure with pulmonary encephalopathy. Methods A total of 70 case type Ⅱ respiratory failure patients with pulmonary encephalopathy were randomly divided into two groups： the obser- vation group and the control group, The control group underwent conventional anti-inflammatory, antitussive and ex- pectorant, maintain airway treatment. The observation group in the control group was given based on noninvasive posi- tive pressure ventilation combined with naloxone, aminophylline treatment. And at the end of ventilation, ventilation, before after treatment in patients with symptoms, vital signs and ventilation changes made correlation. Results In the observation group, the total efficiency is better than that of the control group, PaCO2, combined with naloxone group were significantly decreased, and the differences had statistical significance （P 〈 0.05）. Conclusion Noninvasive posi- tive pressure ventilation combined with naloxone, aminophylline on type Ⅱ respiratory failure with pulmonary en- cephalopathy ideal treatment effect, clinical treatment is safe, it is worthy of further promotion in clinical application.
