机械通气中应用囊内压测定法疗效的系统评价

摘要 目的:评价在机械通气中应用囊内压测定法的疗效。方法:我们通过检索近10年Cochrane Library、Medline、Pubmed、Web of Science、万方数据库、维普中文科技期刊全文数据库、中国学术期刊全文数据库,对纳入研究的方法学进行评价。研究者对纳入文献的质量进行严格评价和资料提取,使用Review manager 5.0软件对符合质量标准的RCT行系统评价。结果:6个RCT共272名患者纳入研究,其中治疗组(使用囊内压测定法)135例,对照组(使用固定注气法)137例,2组患者均行常规对症支持治疗。系统评价结果表明,与对照组相比,治疗组患者8h后气道损伤、8h后误吸呛咳、2d气囊破裂漏气的发生率差异具有统计学意义。结论:囊内压测定法可显著提高患者机械通气的疗效。 Objective: To evaluate the efficacy of using intrasac pressure t in mechanical ventilation Methods: We have searched recent 10 years' Cochrane Library, Medline, Pubmed, Web of Science, CECDB, CQVIP, CNKI in this study. The collected literatures were strictly evaluated, extracted and qualified. Systemic evaluation by Re- view, Manager, 5.0 software on the RCT-line systems that meet quality standards was carried out. Results: Two hundred and seventy-two patients in 6 RCT were included in the study, 135 cases in the treatment group used intrasac pressure measurement, and 137 cases in the control group used constant gas injection. Patients in both groups were also given symp- tomatic and supportive treatments. Meta-analysis showed that, there were significant differences in airway mucosal injury and aspiration induced cough after 8 hours, as well as rupture and leakage of balloon in 2 days between two groups. Conclu- sion: Intrasac pressure measurement can significantly improve the efficacy of mechanical ventilation.
