摘要
目的:研究水合氯醛全麻状态下顺式阿曲库铵对大鼠坐骨神经复合肌肉动作电位(CMAP)的影响。方法:60只SD大鼠随机分为2组,7%水合氯醛腹腔注射麻醉后,实验组腹腔注射顺式阿曲库铵(0.93mg/kg),对照组腹腔注射等体积生理盐水,立即刺激坐骨神经,记录大鼠腓肠肌CMAP变化,刺激间隔5min。结果:麻醉生效后18~48min2组间峰峰值及潜伏期差异有统计学意义(P<0.05)。实验组CMAP变化分3个阶段:诱导期,表现为峰峰值逐步下降,潜伏期延长,持续约10min;抑制期,表现为峰峰值进一步降低,潜伏期进一步延长,之后有恢复趋势,持续约30min;恢复期,表现为峰峰值、潜伏期恢复至用药前水平。对照组CMAP峰峰值与潜伏期均无显著变化(均P>0.05)。结论:顺式阿曲库铵对大鼠腓肠肌CMAP有显著影响,而且呈阶段性变化,准确了解这种变化趋势,有助于术中及时为术者反馈信息,减少神经损伤。
Objective : To study the effects of cisatracurium on compound muscle action potential (CMAP) of sci- atic nerve in rats under general anesthesia by chloral hydrate. Methods: A total of 60 SD rats were divided into two groups randomly. Cisatracurium (0.93mg/kg) was injected intraperitoneally into the rats of the experimental group, and isotonic saline of the same volume in the control group after anesthetized by 7% chloral hydrate, respectively. Sci- atic nerve was stimulated after injection at 5 minutes intervals, and recorded the changes of CMAPs of gastrocnemius muscle. Results: Significant differences existed in peak-to-peak values and latencies between two groups 18 -48 mi- nutes after general anesthesia ( P 〈 0.05 ). The CMAPs could be divided into three phases in the experimental group. Induction phase: the peak-to-peak values reduced and the latencies prolonged gradually. This phase lasted about 10 mi- nutes. Inhibition phase: the peak-to-peak values reduced and the latencies prolonged further, then presented a recovery trend. This phase lasted about 30 minutes. Recovery phase : the peak-to-peak values and latencies recovered to the pre- vious level before injection. The peak-to-peak values and latencies had no significantly changes in the control group (P 〉 0.05 ). Conclusions : The CMAPs of gastroenemius muscles can be significantly impacted by cisatraeurium, and present phase changes. Accurate understanding the trend of variation of CMAPs conduces to gather feedback information for operater during surgery, and thus may reduce nerve injury.
