踝上后倾截骨术37例临床分析

摘要 目的对踝上后倾截骨术新术式临床疗效初步总结与分析。方法2010年9-12月,对37例胫腓骨远端骨折畸形愈合患者,行踝上后倾截骨术新术式手术治疗并随访观察。用AOFAS踝关节评分系统评分。结果37例患者随访6-24个月,骨折全部愈合,愈合时间平均(15.5±3.4)周。完全负重时间平均(15.1±2.4)周。术后12个月AOFAS评分为61-98分,平均(89.5±7.1)分。结论踝上后倾截骨后,距骨相对跖屈,关节间隙相对增宽,有利于关节软骨面恢复,防止关节负重时进一步加重创伤。后倾5°-10°,踝穴稳定性基本不受影响。 Objective To summary and analyze the clinical results of the supramalleolar retroversioflexion osteotomy. Methods Thirty seven patients in our hospital from September to December 2010 with distal tibia and fibula fracture deformity healing were treated with this operation and followed-up observation of patients, The function of the ankles were assemented by AOFAS scoring system. Results Thirty seven patients were followed up for 6 - 24 months, all of them were fractures healed, the mean time was (15.5±3.4) weeks, the mean time of full weight-bearing was (15.1 ±2.4) weeks; The mean score of the ankle function 12 months after operation was (89.5±7.1) parts. Conclusion After supra- malleolar retroversioflexion osteotomy, subastragalar plantar flexion relatively, the joint gap magnified relatively, make a favor to the recovery of the artilago articularis, guard against becoming more serious with joint-bearing, back lean at 5°to 10°,the steady of ankle wortise was almost actorish.
