切口选择对微创全髋关节置换术治疗老年股骨颈骨折疗效的影响 被引量:9

摘要 目的观察前外侧微创入路及后外侧微创入路全髋关节置换术治疗老年股骨颈骨折的临床效果。方法选择股骨颈骨折需行全髋关节置换的老年患者180例,随机分为前外侧微创入路组(A组)、后外侧微创入路组(B组)及常规后外侧入路组(C组),每组60例,比较各组治疗效果。结果 C组的手术切口长度及术中出血量均显著高于A、B 2组(P均<0.05),术后3 d内下地负重例数及3个月内可独立行走例数均少于其余2组(P均<0.05),术后3个月C组Harris评分低于其他2组(P均<0.05),但3 a后3组Harris评分并无显著性差异(P均>0.05),A、B2组各项指标并无显著性差异(P均>0.05)。结论微创术式较常规外侧入路术式可以显著缩短患者手术切口长度,减少术中出血量,患者恢复较快;前外侧微创入路与后外侧微创入路手术效果并无显著性差异,临床医生可根据实际情况选择手术入路。
