益气解毒法治疗多发性肌炎的临床研究

Clinical Study on Polymyositis Treated by Qi-Benefiting and Detoxificating Method

为从临床方面探讨益气解毒法治疗多发性肌炎 (Polymyositis ,PM)的作用及作用机理 ,将 5 3例PM患者随机分为中药加小剂量激素的治疗组 (35例 )与大剂量激素的对照组 (18例 ) ,分别观察治疗前及治疗后 1个月、3个月、6个月患者的肌力、肌压痛、血清肌酶 (CK、LDH、AST)及激素用量与病情复发率、激素副反应发生率等指标 ;治疗前后患者外周血清IL -2、α -TNF、γ -IFN、CD4/CD8、IgG、C3 等的水平。结果 :益气解毒法能部分替代激素 ,顺利撤减激素和治疗PM ,并减少病情的复发及激素副反应的发生 。

In order to investigate the function and mechanism of “Qi-Benefiting and Detoxificating Method'in the treatment of polymyositis(PM),53 cases of PM patients were randomly divided into the group(35 cases)treated with Chinese herbal drugs plus small dose of steroid and the control group(18 cases)treated with large dose of steroid,to observe respectively the indexes of myodynamia,muscular tenderness,serokinase(CK,LDH,AST),steroid dosage,recurrence rate,incidence rate of side effect steroid in the patients before the treatments and one month,three months and six months after the treatments,and to observe the level of peripheral serum IL-2,α-TNF,γ-IFN,CD 4/CD 8,IgG,and C 3 before and after the treatments.The results showed that the “Qi-Benefiting and Detoxificating Method'could replace steroid partially,disuse and deduct steroid smoothly and treat PM,and also could decrease the recurrence rate and occurrence of steroid parallergic reaction,indicating that its functional mechanism could be related to regulation of immunological abnormality in PM patients.
