用MVP方案治疗老年晚期非小细胞肺癌近期疗效和毒性观察

CLINCAL OBSERVATION ON THE SHORT- TERM EFFECT AND TOXICITY OF MVP REGIMEN IN THE MANAGEMENT OF NON-SMALL CELL LUNG CANCER IN ELDERLYS

目的 观察抗肿瘤药物丝裂霉素（ ＭＭＣ）、西艾克（ＶＤＳ）和顺铂（ ＤＤＰ）组成的 ＭＶＰ方案治疗晚期非小细胞肺癌（ＮＳＣＬＣ）的近期疗效和毒性。方法 治疗老年晚期非小细胞肺癌 ５６例，用 ＭＶＰ方案：ＭＭＣ ６ｍｇ／ｍ～２静推第１天，ＶＤＳ ３ｍｇ／ｍ～２静推第１、８天，ＤＤＰ ４０ｍｇ静滴第１～３天。结果该方案近期有效率４６．４％，毒性反应主要为骨髓抑制，Ⅲ、Ⅳ度白细胞下降率３９％，Ⅲ、Ⅳ度血小板下降率１６％，Ⅲ、Ⅳ度恶心呕吐发生率２１％。结论 ＭＭＣ、ＶＤＳ、ＤＤＰ联合治疗非小细胞肺癌有效率高，其毒性患者可以耐受，是临床治疗非小细胞肺癌切实可行的方案。

Objective This study is to observe the short - term effect and toxicity of mitomycin, vindesine and cisplatin (MVP) in the treatment of advanced non - small cell lung cancer (NSCLC) in elderlys. Method 56 elderly patients with advanced NSCLC were treated with MVP regime. Mitomycin was given 6 mg/m～2 iv on day 1, Vindesine 3 mg/m～２ iv on day 1,8, Cisplatin 40 mg iv drip on day 1-3. Results The short - term response rate was 46 .4%. The dose limiting toxicity was neutropenia, which was in grade Ⅲ and Ⅳ in 39% of the treated patients: Grade Ⅲ and Ⅳ thrombocytopenia was observed in 16% and grade Ⅲ and Ⅳ nausea vomiting in 21 %. Conclusions A higher response rate is attainable in elderly patients with advanced NSCLC treated by MMC, VDS and DDP with tolerable toxicity.
