血液灌流对慢性肾衰竭维持性血液透析患者血清瘦素水平的影响

INFLUENCE OF HEMOPERFUSION ON THE SERUM LEPTIN IN MAINTAINANCE HEMODIALYSIS PATIENTS WITH CHRONIC RENAL FAILURE

目的观察血液灌流(HP)对慢性肾衰维持性血液透析(MHD)患者血清瘦素(Leptin)的影响。方法 56例患者随机分为常规血液净化组(HD+HDF)、观察组(HD+HDF+HP),另选健康体检者20例为正常对照组,观察组在常规血液净化治疗基础上每2周加做1次HP,测定HP治疗前后及对照组血清Leptin水平。结果 MHD患者观察组和常规治疗组比较HP治疗前血清Leptin水平差异无统计学意义,但均明显高于对照组;观察组经HP治疗2、4个月后均显著低于常规治疗组。观察组治疗2、4个月后均比治疗前明显下降,治疗4个月后和治疗2个月后相比,下降差异无统计学意义。结论 HP能有效清除MHD患者血液中的Leptin,对改善患者食欲和营养状态可能有益。

Objective To study the influence of hemoperfusion（HP）on the serum leptin in maintainance he- modialysis（MHD） patients with chronic renal failure. Methods Fifty- six MHD patients in author＇s hospi- tal were included, which were divided randomly into routine treatment group and hemoperfusion （HP） group,another 20 healthy ones as the control group. Based on routine treatment, hemoperfusion（HP） group received hemoperfusion each 2 weeks. The serum leptin levels were measured by radioimmunoassay（RIA） before and after HP treatment for 2,4 month. Results Before the treatment with HP, the serum leptin in the routine treatment group and H P group were significantly higher than the control group （P^0. 01）~ Compared with routine treatment group, the serum leptin in HP group was no significant differences （P〉 0.05）. After treatment HP for 2 months, the serum leptin in HP group was significantly lower than those in routine group; but the serum leptin in HP group was no decreased significantly after treatment HP for 4 months（P〉0.05）. Conclusion HP can eliminate serum leptin of MHD patients and may improve the pa- tientgappetite and nutritional status.
