糖耐量及胰岛素水平测定在多囊卵巢综合征中的临床应用

摘要 目的探讨多囊卵巢综合征患者(PCOS)糖代谢和胰岛素水平的改变。方法 173例PCOS患者按体质量指数(BMI)分为PCOS非肥胖组(BMI<25kg/m2)、PCOS肥胖组(BMI≥25kg/m2)和42例正常对照组,各组均做口服糖耐量和胰岛素释放试验。结果与对照组相比,PCOS非肥胖组的空腹和口服葡萄糖后,1、2和3h的血糖差异均无统计学意义(P>0.05),胰岛素水平均有显著升高(P<0.05)。PCOS肥胖组的空腹及口服葡萄糖后,1、2和3h的血糖和胰岛素水平均显著高于对照组和PCOS非肥胖组(P<0.05),差异具有统计学意义。结论 PCOS患者存在糖代谢异常和高胰岛素血症,口服糖耐量试验是用于监测育龄妇女PCOS患者糖代谢的较好指标。 [ Objective ] To investigate the changes in glucose metabolism and insulin levels of patients with polycystic ovary syndrome (PCOS). [ Methods ] According to the BMI, 173 patients with PCOS were divided into PCOS non-obese group (BMI 〈25 kg/m^2), PCOS obese group (BMI ≥ 25 kg/m^2) and 42 women in the normal control group.And do the oral glucose tolerance and insulin release test among groups. [ Resluts ] There were not significant differences between PCOS non-obese group and normal control group in serum glucose levels when fasting and within 1,2 and 3 h after oral glucose (P〉0.05), serum insulin levels in PCOS non-obese group were higher than control group (P〈0.05) .Serum glucose and insulin levels in PCOS obese group were higher than PCOS non-obese group and normal control group when fasting and within 1,2 and 3 h after oral glucose (P〈0.05) . [ Conclusions ] Abnormal glucose metabolism and hyperinsulinema exists in PCOS patients, oral glucose tolerance test is a good indicator for monitoring the glucose metabolism of women of childbearing age PCOS patients.
