留置气囊尿管致男性尿道损伤原因分析与对策

Cause analysis and countermeasures on male urethra injury by inserting a balloon urine catheter

目的了解留置气囊尿管致男性尿道损伤的原因。方法回顾性分析2011年1月～2012年12月自贡市第一人民医院留置气囊尿管尿道损伤的病例资料,分析引起尿道损伤的原因,提出防范对策。结果 2011～2012年共有6例气囊尿管所致的尿道损伤,其中导尿管插入不到位注水致尿道破裂2例,操作粗暴致尿道损伤1例,患者强行拔管致尿道损伤2例,困难拔管致尿道损伤1例。结论气囊尿管留置导尿是有创性、高风险操作,必须严格操作准入制度、履行风险告知制度,完善操作程序、加强培训、规范操作。

Objective To analyse the causes of urethral injury caused by inserting a balloon urine catheter. Methods Collected details of urethral injury cases from January 2011 to December 2012 in Zigong First People＇s Hospital patients with in-dwelling balloon urine catheter, the causes of urethral injury were analyzed, and put forward countermeasures. Results There were six cases of urethral injury, two of them were caused by the inserted catheter not reaching the designated position and causing urethral rupture when inflating the balloon, one patient had a urethral injury caused by operating roughly, two cases were because of forced removal of the urine catheter, another one case urethral injury case was caused when the urine catheter was difficult to remove. Conclusion Indwelling urine catheterisation is invasive and high risk procedure, we must operate a strict admittance system, perform risk disclosure system, improve the operation procedures and strengthen the training and standardized operation.
