超声在桥本甲状腺癌术前诊断中的应用价值

The value of ultrasound in the preoperative diagnosis of thyroid carcinoma

目的分析桥本甲状腺癌的声像图特征,探讨超声在桥本甲状腺癌术前诊断中的应用价值。方法对82例术后有明确病理诊断的桥本甲状腺癌患者与术前进行的超声检查对比分析,总结桥本甲状腺癌的声像图特点。结果桥本甲状腺癌有如下特点:结节多呈低回声,无明显包膜,边缘不规整;多伴有砂砾样钙化;有的局部腺体回声明显减低,伴有弥漫性的砂砾样钙化。结论桥本甲状腺癌在超声图像上有其明显的特征性表现,通过超声检查多可以在术前做出明确的诊断,为临床诊治提供确切的信息。超声检查是此疾病术前诊断的首选。

Objective To analyze the ultrasonographic characteristics of hashimoto′s thyroiditis and thyroid cancer and discuss the value of ultrasound in the preoperative diagnosis of coexisting hashimoto′s thyroiditis and thyroid carcinoma.Methods The ultrasonographic characteristics of 82 patients with hashimoto′s thyroiditis and thyroid cancer were analyzed.Results The Hashimoto thyroiditis coexisting with thyroid cancer had following characteristics: the nodules were usually hypoechoic,irregular-edged,with no obvious capsules,and often accompanied with gravel-like calcification.Parts of some of glands generated significantly-reduced ultronic echo and associated with diffuse calcification.Strip like bloodstream could be seen in multiple nodules.Conclusion Ultrasonic image of Hashimoto′s thyroiditis coexisting with thyroid carcinoma has its obvious features,which made ultrasound the first choice for the preoperative diagnosis of the disease.
