老年冠心病患者B型利钠肽水平的临床分析 被引量:5

摘要 目的分析老年冠心病和非冠心病患者B型利钠肽(BNP)水平与心脏结构和功能的相关性及性别差异。方法收集2010年9月~2012年2月在中国医科大学附属第一医院老年病科住院的,年龄≥60岁的冠心病患者(冠心病组)193例,另选同期住院的非冠心病患者(非冠心病组)198例。观察2组血浆BNP、肾功能、血脂及心脏功能等指标,比较2组BNP水平及其与心脏结构、功能等临床指标的相关性。结果与非冠心病组比较,冠心病组年龄、收缩压、空腹血糖、BNP、左心房内径、室间隔厚度、左心室内径及舒张功能指标明显升高(P<0.05,P<0.01)。BNP水平与心脏结构、功能存在相关性,女性冠心病患者相关性更密切。结论测定BNP水平可为冠心病患者尤其是老年女性冠心病患者心脏结构和功能的预测,提供有价值的参考依据。 Objective To analyze the relation of 13NP level with cardiac structure and function in elderly patients with or without coronary heart disease (CHD) ,and its gender difference. Methods One hundred and ninety-three CHD patients with their age ≥60 years and 198 non-CHD patients admitted to our hospital from September 2010 to February 2012 served as a CHD group and a non-CHD group, respectively,in this study. Their plasma BNP and lipid levels were measured and their renal and heart functions were detected. The relation of plasma BNP level with cardiac struc- ture and function was compared. Results The age, systolic blood pressure, blood glucose level, plasma BNP level,left atrial diameter,interventricular septal thickness,left ventricular diameter, mitral flow velocity E peak were significantly higher in CHD group than in non-CHD group (P〈 0. 05,P〈0. 01). The plasma BNP level was correlated with the cardiac structure and function in CHD patients,especially in female CHD patients. Conclusion Measurement of plasma BNP level can provide the valuable evidence for predicting the cadiac structure and function in CHD pa- tients,especially in elderly female CHD patients.
