摘要
目的比较暴露喉返神经的三种路径,总结手术经验。方法回顾我院180例甲状腺手术,暴露喉返神经的途径分为:上方路径组,中间路径组和下方路径组。对比各组的手术时间和神经损伤。结果三组间比较,上方路径的解剖时间最短,中间路径无神经损伤,各组的出血量无明显差异。结论精细甲状腺背膜解剖法,首选中间途径暴露神经;掌握喉返神经的各种解剖标志,根据甲状腺病变,灵活采取不同路径显露喉返神经。
Objective To compare the three exposed path of recurrent laryngeal nerve （RLN） and summarize the surgical experienee. Methods 180 eases of thyroid surgery in our hospital were retrospectively analyzed, and divided into the upper group, the middle path group and the below path group according to RNL anatomical path. Each group was compared with operative time and nerve damage. Results Among three groups, the anatomical time was the shortest in the upper group. There was no nerve damage in the middle path, and no significant difference in the haemorrhage of groups. Conclusion Meticulously dissect the dordal membrane of thyroid, fist choose the middle path, master various anatomic landmarks of RLN, and Flexible to take different paths to reveal RLN aeeording to thyroid lesions.
