原发性脾脏肿瘤12例临床病理研究

Clinicopathological Study of the Primary Tumors of the Spleen

探讨脾肿瘤的临床病理特点。方法 回顾分析 12例脾肿瘤的临床病理资料。结果 12例脾肿瘤中血管瘤 5例 ,错构瘤 2例 ,表皮样囊肿 1例 ,恶性淋巴瘤 2例 ,恶性纤维组织细胞瘤 1例 ,血管肉瘤 1例。 8例良性瘤术后均无复发 ,4例恶性瘤于术后8月内死亡。结论 脾肿瘤较少见 ,临床表现无特异 ,B超、CT、MRI等影象检查有助于发现病变 ,确诊有赖于病理检查 ,手术是主要治疗方法 ,脾恶性肿瘤预后差。

Objective To study the clinicopathologic characteristics of the primary tumors of the spleen.Methods Twelve cases of primary tumors of the spleen were studied by clinicopathology in combination with the review of the literature.Results Twelve cases of primary tumors of the spleen include 5 cases of hemangioma,2 case of hamartomas,1 case of epidermoid cysts,2 cases of malignant lymphomas,1 case of malignant fibrous histiocytoma and 1 case of angiosarcoma.Six benign cases were alive without recurrence arter operation,2 benign cases died of other diseases,whereas 4 malignant cases all died within 8 monthes after treatment.Conclusions Primary tumors of the spleen are rare,imaging examinations (BUS,CT,MRI)are contributive to finding the lesions,and diagnosis and differential diagnosis depend on pathology.Surgical resection is the first choice in the treatment.The prognosis of malignant cases is bad.
