小儿腹股沟斜疝手术中应用利多卡因持续静脉泵注联合氯胺酮麻醉效果观察 被引量:1

摘要 目的总结利多卡因静脉持续泵注联合氯胺酮静脉麻醉在小儿斜疝手术中的应用体会。方法将60例ASAⅠ~Ⅱ级择期行腹股沟斜疝手术患儿随机分为利多卡因联合氯胺酮组(L组)和氯胺酮组(K)组各30例。记录苏醒期恶心、呕吐、幻觉、烦躁、谵妄、肢体乱动、呼吸抑制等不良事件例数。结果与T0相比,2组患儿麻醉后及术中各时段MAP都明显升高、HR明显加快。K组患儿T2~T4时MAP高于L组(P<0.05),HR也快于L组(P<0.05)。2组患儿SpO2值均维持在95%以上。术后患儿苏醒时间L组为(31.5±5.5)min明显短于K组(51.0±2.5)min(P<0.05)。L组氯胺酮用量为(118.5±10.5)mg,明显少于K组的(216.5±13.5)mg(P<0.05)。2组患儿麻醉过程都平稳,术中影响手术体动例数L组明显少于K组(P<0.05);苏醒期躁动及肢体乱动例数L组多于K组(P<0.05),其他不良事件发生例数2组比较差异无统计学意义(P>0.05)。结论利多卡因持续静脉泵注联合氯胺酮静脉麻醉用于小儿腹股沟斜疝手术,可减少氯胺酮用量,诱导迅速、苏醒较快,术中血流动力学稳定。
