摘要
目的观察起搏治疗对持久缓慢心律失常伴心脏明显扩大者的疗效。方法对9例持久缓慢心律失常伴心脏明显扩大者行永久起搏治疗 ,并作临床及超声心动图随访。结果起搏术后3月心衰症状完全消失。LVEDD术前61±3mm术后6月及12月分别缩小至55±3mm(P<0.01)及51±4mm(P<0.05),LVESD术前40±4mm ,术后12月缩小至33±5mm(P<0.05) ,LVEF无改变。结论长期缓慢心律失常可使心脏扩大及心功能受损 ,这类患者用起搏治疗能有效地改善心功能。
Objective To observe the long-term therapeutic efficacy of pacemaker implantation in bradycadia patients with dilated left ventricle and heart failure. Methods Pacemakers were implanted in 9 bradycadia patients with dilated left ventricle and heart failure. Serial clinical and echcardiography evaluation were carried out respectively. Results 3 months after the implantation, the symptoms of the heart failure disappeared in all the cases. The mean left ventricular end diastolic diameter (LVEDD) and the left ventricular end systolic diameter (LVESD) were reduced from 61+3 mm and 40+4 mm before the implantation to 55+3 and 33+5 after the implantation respectively (P<0.05). No change was in LVEF. Conclusion Regression of the dilated left ventricle and heart after the pacing implied the existence of bradycardia-induced cardiomyopathy which could be cured by pacemaker implantation.
