从炎症细胞因子研究清气化痰汤对慢性阻塞性肺疾病急性加重期的作用机制

Study on Mechanism of Qingqi Huatan Decoction in Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease by Inflammatory Cytokines

目的观察在西医常规治疗基础上,清气化痰汤治疗慢性阻塞性肺疾病(COPD)急性加重期患者的临床疗效,并从炎症细胞因子角度研究清气化痰汤的作用机制。方法将COPD急性加重期患者随机分为治疗组和对照组各40例,2组均采用西医常规治疗,治疗组在此基础上加服清气化痰汤,疗程10 d。观察治疗前后2组临床疗效及相关炎症细胞因子白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平。结果 2组经治疗后,血浆炎症细胞因子IL-8、TNF-α水平下降,与治疗前比较,差异有统计学意义(P<0.01);治疗后组间比较,差异有统计学意义(P<0.01)。治疗组临床疗效优于对照组(P<0.01)。结论在西医常规治疗基础上加用清气化痰汤治疗COPD急性加重期患者,可改善临床症状及体征,其作用机制之一可能在于通过降低炎症细胞因子水平来减轻气道炎症。

Objective To observe the clinical effect of Qingqi Huatan decoction in treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） by reducing the level of inflammatory cytokines in plasma.Methods Eighty patients with AECOPD were randomly divided into the treatment group and the control group.The treatment group were treated by Qingqi Huatan decoction（Chinese medicine decoction of clearing away pathogenic factors and resolving phlegm） and conventional therapy as same as the control group.The treatment course was 10 days.The therapeutic effect and the level of IL-8 and TNF-α in plasma of two groups after treatment were compared.Results After treatment,the level of IL-8 and TNF-α in plasma of the two groups decreased（P 0.01）.There was significant difference between the two groups（P 0.01）.The therapeutic effect of the treatment group was superior to the control group（P 0.01）.Conclusion Qingqi Huatan decoction can improve the symptoms of AECOPD patients,and the mechanism may be relieving inflammation by decreasing the level of inflammatory cytokines in plasma.
