摘要
目的探讨低分子肝素钠在治疗重度慢性阻塞性肺疾病(COPD)患者急性加重期中对凝血功能和肺功能的影响。方法选择2010年10月~2012年10月来我院就诊的重度COPD患者112例作为研究对象,将患者分为两组:对照组和观察组,每组56例,分别给予常规治疗、常规治疗+低分子肝素钠治疗,对两组患者治疗前后的凝血功能及肺功能进行比较。结果观察组治疗后与治疗前比较PT、APTT、TT均显著升高,而FBG、DD均显著降低(P<0.05),观察组治疗后与对照组比较,差异有统计学意义(P<0.05)。对比对照组及观察组治疗后与治疗前,其FEV1均有显著的升高,差异有统计学意义(P<0.05);观察组治疗后FEV1明显高于对照组,差异有统计学意义(P<0.05)。结论常规治疗同时配合低分子肝素钠等抗凝治疗措施可以大大提高重度COPD患者的治疗效果,对病情控制及预后等均具有重要的意义。
Objective To investigate the effect of low molecular weight heparin in the treatment of severe chronic obstructive pulmonary disease(COPD) patients with acute exacerbation on blood coagulation function and pulmonary function in the period. Methods 112 severe COPD patients from 2010 October to 2012 October in our hospital for treatment were selected,the patients were randomly divided into 2 groups:the control group and the observation grnup,56cases in each group,they were given conventional treatment,the conventional treatment and low molecular heparin treatment,we compared blood coagulation function and pulmonary function of 2 groups before and after treatment. Results The observation group after treatment compared with before treatment,PT,APTT,TT increased significantly,while FBG,DD were significantly decreased(P 〈 0.05),the observation group after treatment compared with the control group also has significant differences(P 〈 0.05).Compared with the control group and the observation group before treatment and after treatment,the FEV1 were all significantly increased,the difference was statistically significant(P 〈 0.05);the observation group after treatment,FEVl was significantly higher than the control group,the difference was statistically significant(P 〈 0.05). Conclusion Routine treatment combined with low molecular heparin sodium anticoagulation therapy can significantly improve the treatment effect of the patients with COPD,which has significance for the control of disease and prognosis.
