摘要
目的探讨多形性日光疹(PLE)及其家族史与HLA-DQB1等位基因的相关性。方法应用聚合酶链反应-直接测序(polymerase chain reaction sequence-based typing,PCR-SBT)的方法对110例PLE患者和170例健康对照进行HLA-DQB1等位基因的检测。结果共检测出14种HLA-DQB1等位基因,其中HLA-DQB1*0302和DQB1*0601等位基因与PLE呈正相关(OR=7.485,Pc<10-3;OR=14.153,Pc<10-4),而DQB1*0201等位基因与PLE呈负相关(OR=0.354,Pc=0.0280)。HLA-DQB1*0302(OR=0.101,Pc<10-4)与无家族史的PLE呈正相关,而DQB1*0601(OR=0.069,Pc<10-3;OR=0.075,Pc=0.0084)与有和无家族史的PLE均呈正相关,但DQB1*0201(OR=5.500,Pc=0.0056)与无家族史的PLE呈负相关。结论 HLA-DQB1*0302和DQB1*0601可能是云南汉族PLE的易感基因或与实际的易感基因连锁,而HLA-DQB1*0201可能为云南汉族PLE的保护性基因;DQB1*0302可能为无家族史PLE患者的促进因素,而DQB1*0201可能为无家族史PLE患者的保护因素。
Objective To explore the association of HLA-DQB1 alleles with polymorphic light eruption in Han Nationality in Yunnan. Methods PCR-SBT method was used to examine the HLA-DQB1 allele in 110 PLE patients and 170 Healthy controls in Han Nationality in Yunnan. Results Compared with the control group, allele frequen-cy of DQB1 * 0302 and DQB1 * 0601 ( OR = 7. 485 ,Pc 〈 10^-3 ; OR = 14. 153 ,Pc 〈 10^ -4 ) were significantly increased in PLE patients, while the frequency of DQB1 * 0201 was significantly reduced in case group ( OR = 0.354, Pc = 0. 028 0). HLA-DQB1 * 0601 ( OR = 0.075,Pc = 0.008 4) was positively associated with positive family history. DQB1 * 0302 and DQB1 * 0601 ( OR = 0. 101,Pc 〈 10^-4, OR = 0. 069,Pc 〈 10^-3, respective-ly) were positively and DQB1 * 0201 (OR = 5. 500,Pc = 0. 005 6) was negatively associated with negative family history. Conclusion HLA-DQB1 * 0302 and DQB1 * 0601 may be the susceptibility genes or linkage disequilibrium with the real susceptible gene. HLA-DQB1 * 0201 may be the protective gene of PLE of the Han Nationality in Yunnan. DQB1 * 0302 may be a contributing factor in PLE patients with negative family history, DQB1 * 0201 may be protective factors in PLE patients with negative family history.
