摘要 目的:总结国内外关于肿瘤淋巴管生成的研究进展。方法:应用PubMed和CNKI期刊全文数据库检索系统,以"肿瘤、淋巴管生成和淋巴转移"为关键词,检索2000-01-2010-11的相关文献,共检索到英文文献591篇,中文文献655篇。纳入标准:1)肿瘤淋巴管形态学特点与肿瘤临床病理因素的关系;2)肿瘤淋巴管生成的机制;3)肿瘤淋巴管生成的分子调控机制。根据纳入标准分析31篇文献。结果:肿瘤组织内部淋巴管可能是功能缺陷的,但肿瘤组织内部增多的淋巴管增加了肿瘤淋巴转移的概率,肿瘤组织内部淋巴管密度与肿瘤淋巴结转移相关。肿瘤组织中有功能的淋巴管多数位于肿瘤组织周围,肿瘤组织周围淋巴管密度与肿瘤淋巴结转移相关,肿瘤组织周围淋巴管可能在肿瘤淋巴转移过程中发挥了更为重要的作用。肿瘤淋巴管生成的机制十分复杂,目前尚无定论,有研究认为肿瘤新生淋巴管主要来源于组织中既存的淋巴管内皮细胞。在肿瘤淋巴管生成过程中,许多因子如VEGF-C、VEGF-D、VEGF-A、PDGF、HGF、IGF、FGF-2和Ang等都直接或间接参与其中,但多数研究认为VEGF-C和VEGF-D是肿瘤淋巴管生成中最重要的调控因子。结论:肿瘤淋巴管生成与肿瘤淋巴转移密切相关,抑制肿瘤淋巴管生成能为肿瘤治疗开辟一条新的途径。 OBJECTIVE: To sum up the research progressions related to tumor Iymphangiogenesis. METHODS: The literature was searched with "tumor,lymphatic metastasis,lymphangiogenesis" as the key words from 2000-01 to 2010-11 by using PubMed and CNKI retrieval systems. Total 591 English and 655 Chinese papers were obtained. Chioce criteria: 1) Mechanisms of tumor lymphangiogenesis. 2)Molecular regulation of tumor lymphangiogenesis. 3)The relationship between tumor lymphangiogenesis and tumor clinicopathological factors. Totally 31 papers were finally analyzed according to the chioce criteria. RESULTS: Intratumoral lymphatic vessels may functional defect, but the increased numbers of lymphatic vessels in the tumor provid more opportunities for lymphatic metastasis. The intratumoral lymphatic vessels density (ILVD) is closely associated with lymphatic metastasis. The functional lymphatics which are often located around the tumor tissue played a more important role in tumor lymphatic metastasis. The peritumoral lymphatic vessels density (PLVD) is also closely associated with lymphatic metastasis. The mechanism of tumor lymphangiogenesis is complicated and has no definite conclusion. It has been reported that the neonatal tumor lymphatics originated from the original lymphatic endothelial cells. Many factors have involved in the regulation of tumor lymphangiogenesis, such as VEGF-C, VEGF-D, VEGF-A, PDGF, HGF, IGF, FGF-2 and Ang, but most of studies have suggested that VEGF-C and VEGF-D are the most important regulation factors. CONCLUSION: Tumor lymphangiogenesis is closely related to the lymphatic metastasis, interventions to inhibit tumour lymphangiogenesis can be applied as a novel tumor therapy.
