无创通气治疗50例拒绝气管插管的AECOPD并SRF的疗效分析 被引量:14

摘要 目的探讨无创通气(NIV)治疗拒绝气管插管(DNI)的慢性阻塞性肺疾病急性加重期(AECOPD)并严重呼吸衰竭(SRF)的效果及其影响因素。方法对50例AECOPD并SRF患者满足气管插管有创机械通气标准但DNI患者,予以常规治疗及NIV治疗,根据治疗效果分为成功组和失败组。结果 50例患者中有36例(72.00%)治疗成功,14例(28.00%)治疗失败。治疗成功组36例患者治疗1~2h后动脉氧分压(PaO2)、格拉斯哥昏迷评分(GCS)均上升(P<0.05),4h后动脉二氧化碳分压(PaCO2)与pH值均明显好转(P<0.01)。治疗失败组14例患者治疗1~2h、4h后血气(pH值、PaCO2和PaO2)和GCS也有所改善(P<0.05),但经1~3d后病情趋于恶化,9例(18.00%)在院死亡。与治疗成功组比较,治疗失败组患者年龄大、病程长、GCS小、动脉血pH值和PaO2低以及PaCO2较高。结论 NIV治疗AECOPD并SRF的DNI患者有一定效果。
