医用粘合胶制动对兔血管吻合后内膜和中膜增生的影响 被引量:4

Influences of Medical Glue Braking on Intima and Media Hyperplasia after Vascular Anastomosis in Rabbits

摘要 目的观察医用粘合胶制动对兔血管吻合后吻合口部位内、中膜增生的影响。方法取2.5~3.0 kg新西兰大白兔40只随机分为两组,实验组20只,切断左侧颈总动脉后再吻合,将吻合口悬空,滴医用粘合胶于吻合口处及吻合口前后各1 cm,完全覆盖而不压迫吻合口,待粘合胶凝固后,观察血管通畅及搏动状况后,逐层缝合。对照组20只,同法切断兔颈总动脉后吻合血管断端,未涂抹医用粘合胶。术后4周进行血管造影,取出血管,观察吻合口内膜、中膜增生情况。结果术后两组均见内膜及中膜的增生,内膜可见大量细胞及丰富基质。实验组内膜、中膜厚度均较对照组小(P<0.05)。结论血管吻合口涂抹医用粘合胶可以减轻兔血管吻合口内膜及中膜增生,从而预防血管吻合口狭窄。 Objective To observe influences of vascular braking with medical glue outside the vascular anastomotic parts on membrane hyperplasia in rabbits. Methods Forty New Zealand rabbits (2.5 -3.0 kg) were randomly divided into glue group (n = 20) and control group (n = 20). Rabbits in glue group were cut off the left common carotid artery, sewed it later, and dropped medical glue on and 1 cm before and after anastomotic stoma with impending anasto- motic stoma. Medical glue was completely covered without oppressing anastomotic stoma. Conditions of vascular patency and vascular pulsing were observed after glue solidified, then layered suture was performed. Rabbits in control group were transected the common carotid artery by the same way, and were sewed the haemal broken ends without dropping the biological glue. The angiography was performed for the two groups 4 weeks after operation; blood vessels were taken out to observe the hyperplasia of intima and media of anastomosis stoma. Results Intima and media hyperplasia occurred in two groups after operation, and numerous cells and abundant matrix could be observed. The thicknesses of intima and media in glue group were less than those in control group (P 〈 0. 05). Conclusion Gluing cyanoacrylate around the anastomosis in rabbits could inhibit the intimal and median hyperplasia, so it may prevent the stenosis after vascular anastomosis.
